AGING AND DISABILITY RESOURCE CENTER SPECIALIST II

AGING AND DISABILITY RESOURCE CENTER SPECIALIST II- Hourly


MINIMUM QUALIFICATIONS:

·        At least one year experience in Long Term Care for elderly and/or disabled adults, medical social work, or geriatrics/gerontology, desired.

·        Knowledge/experience of the aging network desired.

·        Genuine interest in the Mission of Area Agency on Aging 3.

  • Ability to accurately obtain and enter data in various data systems.

  • Ability to use telephone system with professional business procedures to include courteous, efficient telephone skills, and prompt routing of telephone calls.

  • Excellent written and verbal communication skills.

·        Proficient in computer skills.

·        Ability to adapt to changing job duties and multi-task in busy office setting.

  • Ability to work with minimal supervision, problem solve and complete assignments efficiently and accurately.

·        Must have a telephone.

·        Must have a valid driver's license, access to an automobile, and automobile insurance meeting the minimum requirements of state law.

·        Final candidate must agree to a BCII Criminal Records Check, which includes fingerprinting and pre-employment Drug Screen at the expense of Area Agency on Aging 3.

 

 

ESSENTIAL FUNCTIONS:

  • Route calls to the proper staff member and takes accurate messages, when applicable.

·        Assist individuals through the Long Term Care Questionnaire in accessing both Medicaid and Non-Medicaid funded community-based long-term services and supports through the ADRN (Aging and Disability Resource Center) via phone, e-mail, web site and in person.

·        Link individuals through Support Navigation to resources, services, and opportunities that maintain the greatest degree of independence and assists individuals in determining and securing the most appropriate setting for long-term care.

·        Find appropriate referral sources for individuals and assist with locating public assistance programs.

·        Assist individuals and their families in understanding Medicaid requirements and regulations.

·        Verify in Ohio Benefits and MITS an individual’s Medicaid status.

·        Follow-up with collection of outside information needed to obtain Medicaid approval for Community Options individuals via phone or in person.

·        Collect initial intake information from individuals seeking information and services.

·        When appropriate, refers to other systems, coordinating and collaborating with both formal and informal support systems.

·        Complete detailed documentation on Support Navigation activities in OBLTSS system.

·        Efficiently and effectively schedule assessments with Assessment Department Staff.

·        Clarifies for individuals their community-based long-term care alternatives to nursing home placement.

·        Provides knowledge, advocacy, coordination, and linkage to individuals regarding community-based long-term care programs.

·        Conduct outreach activities to increase awareness of Agency on Aging on Aging 3 services amongst targeted population.

·        Adheres to privacy and HIPAA regulations.
