Internal Opening - Care Transition Supervisor-FT

We have the following newly added internal additional position available. If you are interested, please let me know by the end of day Monday December 4th.


MINIMUM QUALIFICATIONS:

· Current RN or LSW licensure in the State of Ohio.

· One year supervisory experience preferred.

· Two years prior experience in Long Term Care/ Home and Community Based Services for elderly and/or disabled, home health care, medical social work, or geriatrics/gerontology preferred.

· Must have a valid driver's license, access to an automobile, and automobile insurance meeting the minimum requirements of state law.

· Genuine interest in the Mission of Area Agency on Aging 3.

· Proficient in computer skills.

· Must have a telephone.

  • Final candidate must agree to a BCII Criminal Records Check, which includes fingerprinting and pre-employment Drug Screen at the expense of Area Agency on Aging 3.

· It is the sole responsibility of the employee to keep up with all required licensure (Examples RN, LSW), including continued licensure renewals throughout your employment with Area Agency on Aging 3.

ESSENTIAL FUNCTIONS:

  • Administer management practice, policy and procedures. Provide leadership and assistance to Care Transitions staff.

  • Responsible for initiating quality improvements, problem resolutions and innovations in the Care Transitions department.

  • Provides guidance and training to Care Transitions staff.

  • Complete Legislative visits with staff and individuals for the agency.

  • Adheres to HIPAA regulations.

· Work/job performance location and scheduled hours will be determined by immediate Supervisor/Vice President/Chief Operations Officer/Chief Executive Officer.

  • This position is directly responsible to the VP of Program Operations/Chief Operations Officer/Chief Executive Officer.

  • Performs other related duties as assigned by the VP of Program Operations/Chief Operations Officer/Chief Executive Officer.
