Assessment Scheduler-Hourly non-exempt

The Assessment Scheduler is a full-time position dedicated to supporting the agency’s Assessment team with efficient scheduling of assessments for agency programs. The Assessment Scheduler reports to the Aging and Disability Resource Center Supervisor. The primary functions of this role include scheduling assessments based on primary and secondary county assignments, placing reminder calls to individuals, and rescheduling assessments as needed.

MINIMUM QUALIFICATIONS:

  • High School Diploma or Associates degree required, Bachelor's degree in Social Work, Public Health, Education, or related field preferred.

  • Experience working directly with the aging population and persons with a disability.

  • Understanding of Medicaid waivers.

  • Knowledge of community resources.

  • Good oral and written communication skills.

  • Proficient in computer skills.

  • Must have a valid driver's license, access to an automobile and automobile insurance meeting the minimum requirements of state law.

  • Final candidate must agree to a BCII Criminal Records Check, which includes fingerprinting and pre-employment Drug Screen at the expense of Area Agency on Aging 3.

  • It is the sole responsibility of the employee to keep up licensure, if any, (i.e.: RN, LSW, RD), including continued licensure renewals throughout employment with Area Agency on Aging 3.

ESSENTIAL FUNCTIONS:

  • Schedule assessments with required information needed for assessment in calendar once individuals have completed the intake process with ADRC.

  • Ensure scheduling occurs within ODA required timeframes.

  • Verify Medicaid status in Ohio Benefits/MITS systems.

  • Place reminder calls to individuals at minimum 24 hours prior to their scheduled assessment.

· Work/job performance location and scheduled hours will be determined by immediate Supervisor/Vice President Aging in Place/Chief Operations Officer/Chief Executive Officer.
