MINIMUM QUALIFICATIONS:
Must hold a Master of Social Work Degree from an accredited program.
Licensure required: Licensed Independent Social Worker (LISW) in the state of Ohio.
Must maintain necessary documents/credentialing to meet requirements for the practice of mental/behavioral health therapy in Ohio.
One-year experience providing mental/behavioral health services to older adults in Long Term Care or Home and Community Based settings preferred.
Genuine interest in the Mission of Area Agency on Aging 3.
Strong oral and written communication skills.
Ability to work cooperatively with external and internal referral sources and with coworkers.
Experience with EHR documentation preferred.
Adheres to consumer privacy and HIPAA regulations.
Must have a valid driver's license, access to an automobile and automobile insurance meeting the minimum requirements of state law.
Final candidate must agree to a BCII Criminal Records Check, which includes fingerprinting and pre-employment Drug Screen at the expense of Area Agency on Aging 3.
It is the sole responsibility of the employee to keep up licensure, if any, (i.e.: RN, LSW, RD), including continued licensure renewals throughout employment with Area Agency.
DESIRED QUALIFICATIONS:
· Bilingual (Spanish/English) fluency.
PHYSICAL REQUIREMENTS
Required to stand and sit; ascend and descend ramps and/or stairs; communicate, both in person and by telephone/cell phone/headset. Sitting for extended periods is common. Extensive use of office equipment and computers requiring repetitive motion and substantial movements (motions) of the wrists, hand, and/or fingers. Ability to lift 20 pounds to carry, push, pull or otherwise move objects. Required to operate a motor vehicle.
ESSENTIAL FUNCTIONS:
· Conducts a face-to-face comprehensive assessment with the consumer/family system as a basis for determining the consumer's level of care/level of need, eligibility for available community resources and long-term care options.
Provide direct mental/behavioral health services in a respectful, compassionate, and empathic manner utilizing a person-centered and trauma-informed approach.
Excellent clinical skills, including the ability to collaboratively develop treatment plans with individuals served.
Provide mental/behavioral health services in a culturally responsive manner to individuals from various backgrounds and cultures.
Accurately assess, diagnose, and determine appropriate level of care for individuals’ mental health needs.
Ability to work independently while utilizing evidence-based and strength-based therapeutic interventions.
Maintain timely documentation for mental/behavioral health services according to Area Agency on Aging 3 policies, contracts, and or grants.
Ensure compliance with Federal, State, and Local laws, regulations, codes, and or standards.
Work/job performance location and scheduled hours will be determined by Clinical Director of Mental and Behavioral Health Outreach Services/Vice President of Community Living Services/Chief Operations Officer/Chief Executive Officer.
This position is directly responsible to the Clinical Director of Mental and Behavioral Health Outreach Services/Vice President of Community Living Services/Chief Operations Office/Chief Executive Officer.
